It is your responsibility to know what your insurance benefits are. As much as we like to be able to help you with this, there are too many plans for us to know all of them. Please check with your benefit provider to see if a service is covered, and if we are an in network provider. You are responsible for all deductible amounts and co-pays. All copays are to be paid at time of office visit. An additional $15 charge will be added to any copay not paid at time of service.
If you have an HMO insurance policy, you MUST choose us as your Primary Care Physician (PCP). If we are not registered as your PCP with your insurance we will have to reschedule your appointment until the changes have been made.
